    I accidentally overpaid CBL Path through my Flex Spending Account for laboratory charges to analyze tissue samples that ******************************* requested.Invoices received:Account *********** - $367.80, reduced to $252.17 due to insurance adjustments and write-offs, and payments.Account *********** - $286.80, reduced to $250.92 due to insurance adjustments and write-offs, and payments.Total owed = $503.09 Payments:6/21/24 - FSA paid $634.00 6/26/24 - FSA card payment of $250.92 Total paid = $884.92 Refund required = $381.83 I called them with my FSA representative after the overpayment was processed and they indicated that they would send a refund check back to my FSA for $381.83 at the beginning of the following month when I called in June. A check was not sent and the call notes were not entered into their system.I called again later in July and they told me they would send the refund but no refund was issued. I called and talked to ******** on 8/14/24 and she indicated that the $250.92 card payment would be refunded to my card and they would send a check for $130.91. Nothing was refunded to my card. They did send a check for $86.53, but made the check out to CBL Path Inc. I deposited it anyway, but CBL Path issued a stop payment on the check.I called CBL Path on 9/9/24, and left a message indicating that they need to refund my money ASAP and asking for a call back. No one returned my call.No refund has been issued to date and every time I call in they tell me they will issue a refund, but never do. Can you help get them to send me the refund they owe to me so I can use my FSA funds?Thanks,***************************

    The patient's Insurance adjudicated 2 claims for the patient applying a total of $774.40 to the patients deductible. The patient paid a total of $1,036.43 leading to an overpayment of $262.03 (of which $250.92 was refunded via credit card on 9/9/24. The remaining refund for $11.11 will be sent as a separate refund check

    The check for $86.53 was an additional check mailed to CBLPath from the patients ***.We were advised by the *** to either mail the check back to them or to the patient. We simply mailed the check back to the address. CBLPath did not stop payment on the check, the members *** stopped payment as it was an overpayment.

    I have made contact with the patient advising her of the credit card refund as well as the refund for $11.11 to be mailed via check. I advised the patient that I will include all back up documentation with the check.
