    On 12/4 I saw ** ***** regarding dmv drivers license renewal paper work needs and a eye exam .The state has asked for a letter in addition the the state form for the renewal to occur. I visited office on 12/4 and was told it would be 3 or 4 day and thay would call on Wednesday 12/11 i called office as no one contacted me , was told they had left a note , on Friday 12/13 called again ,again was told they would leave a message , I called again on Tuesday 12/17 , at this point not once has Concord eye care responded, Ion this is asked for a manager, one was not available , I was told they would leave a message and the manager would get back to me , as of 12/19 3pm est Concord eye care has not responded This has left me with no drivers license and is creating great hardship This all occurs at the Concord hospital location

    5/11/23 husband had cataract surgery for new lenses. We paid $2695.00 to have toric lenses . He had surgery to find out a week later.they put wrong lense in his eye. Surgeon could not redo eye because now it is healed. Next surgery they put in the right lense. They swiped.my *** flex.card for $1000 Now *** flex needs receipt for the thousand. We were refunded $695 on my flex card and $800 on our credit card. Because of wrong lense. There is no receipt because the person Teri (billing person) doing the billing made errors. She did not only did not supply anything as to receipt but messed the paper work up so he did not get the right lense. I have tried 2 months to get this resolved from concord Eyecare. I have worked with Pat V*** and Suzanne in their billing. *** Flex now has stop the use of my card until I show receipt for 5/11 surgery. Again today with no assistance from concord eye card. Please help me.

    04/27/2023       Voucher ******- this shows the payment of $1,000 was taken from the patient

    05/11/2023       Voucher ******- this shows the payment of $685 was moved from the 04/27/2023 voucher and then refunded back to the card on 05/25/2023 (for left eye )

    06/08/2023       Voucher ******- this shows the payment of $315 (balance of 04/27/2023 voucher ) was moved to this voucher and applied to the lens for right eye

    The copy of the 06/08/2023 voucher should be the receipt the patient would need for her flex card.
